If massage can “improve” any tissue — unknown — one way it might do it is through simple hydraulics: physically pumping tissue fluids around, and/or stimulating the circulation of blood and lymph. I won’t get into the evidence about it here. Suffice it to say that it might be true, and if it’s true then it may not much matter if the process is uncomfortable. While gentler massage may feel pleasant and satisfying, it is possible that more biological benefits can only be achieved hydraulically — whether it’s comfortable or not. This is even more plausible because of trigger points: it’s likely that the tissue fluids of a trigger point are quite polluted with waste metabolites, and the need for flushing is greater, but it’s especially uncomfortable to squish those polluted patches of tissue.

I don’t know of a massage textbook written during the last 100 years that does not attribute Swedish massage to Peter Henry Ling (1776-1837), a Swede. Many proponents claim that foot reflexology can cleanse the body of toxins, increase circulation, assist in weight loss, and improve the health of organs throughout the body. Others have reported success in treating earaches, anemia, bedwetting, bronchitis, convulsions in an infant, hemorrhoids, hiccups, deafness, hair loss, emphysema, prostate trouble, heart disease, overactive thyroid gland, kidney stones, liver trouble, rectal prolapse, undescended testicles, intestinal paralysis, cataracts, and hydrocephalus (a condition in which an excess of fluid surrounding the brain can cause pressure that damages the brain). Some claim to "balance energy and enhance healing elsewhere in the body." [2] One practitioner has even claimed to have lengthened a leg that was an inch shorter than the other. There is no scientific support for these assertions.
Similar in style to Swedish massage—one of the most popular forms of relaxation massage—deep tissue massage targets the innermost layers of the body’s muscles, tendons, and fascia (the densely woven tissue that connects parts inside the body). Deep tissue massage can also provide relaxation, but massage therapists who specialize in deep tissue typically use much stronger pressure.
Research has shown just 45 to 60 minutes of deep tissue massage can lower blood pressure, reduce stress hormone levels, and decrease heart rate. A study from the University of Maryland found deep tissue massage was even more effective for treating chronic pain than some other medical treatment options. One of the most common culprits for chronic pain is inflammation in the body’s soft tissue, and by increasing blood flow throughout the body, this massage technique is often able to reduce that inflammation. Massage therapists may also work to loosen clusters of tight tissue resulting from physical stress, providing further relief for tense muscles.

In massage therapy, so much can be achieved while inflicting only good pain on patients that bad pain must be justified by vivid, quick, and somewhat lasting benefits — which is a high bar to clear. All health care practices must be justified by benefits. As risk and pain and expense increase, the benefits must also. There is simply no point in tolerating — and paying for — painful treatment without an obvious return on the investment.
Recovery. Therapeutic massage helps the body recover from the stresses of strenuous exercise, and facilitates the rebuilding phase of conditioning. The therapist might use Swedish massage to stimulate circulation of blood and lymph fluids, and trigger point therapy to break down adhesions (knots in the muscles), and stretching to increase the range of motion. Other techniques could include myofascial release, craniosacral therapy, lymphatic drainage and orthopedic assessment. The therapist should also have a good foundation in hydrotherapy modalities including cryotherapy and thermotherapy, which can help with recovery, repair and healing processes.

Swedish massage is proven to lower blood pressure and reduce stress, according to the International Journal of Preventative Medicine, as well as to relieve depression and anxiety and aid in recuperation from chronic illness. Swedish massage is also referred to as classical massage, and — contrary to popular understanding — does not originate from Sweden. There are five main strokes, or movements, that make up a Swedish massage: effleurage, friction, petrissage, tapotement and vibration. Each technique was created to help soothe, stimulate, soften, and rejuvenate muscles and other soft tissue.

Fibromyalgia syndrome is a common and chronic disorder characterized by widespread pain, areas of tenderness, and a number of other symptoms. While massage can be an effective way to manage fibromyalgia pain, most studies done on the effects of massage on fibromyalgia symptoms only showed short-term benefits of massage. Only one study proved to have long-term benefits.9 Though more studies are needed to confirm the positive effects of massage on this condition, you could go to a registered remedial therapist for massage therapy to help with the pain.

There’s rarely any justification for extremely painful massage, unless it clearly produces a better result than gentler treatment — which is rarely clear!1 It is possible that a few “brutal” deep tissue massages could do the trick where gentler treatment would fail — but there is no way to know this in advance, and massage is expensive stuff. This modality was first developed by a Swedish doctor, Dr. Per Henrik Ling, though it possesses more than just Swedish influence. Deep tissue technique uses very little to no lubricant so that the muscles can be hooked or grabbed, thereby stretching and lengthening them, and to separate adhered muscle compartments. Strokes will be considerably slower and possibly shorter as the therapist waits for a slow release of tension. Some areas may be skipped so more time can be spent on specific areas of need. Doing this provides better alignment of the muscles and less restriction in the joints, thereby improving their movement and function. It is a massage in which the primary goal is less about general relaxation and more about promoting change in the actual structure of the body.
Although a lot of Bastian 2014 is certainly relevant to the concept of “good pain,” strictly speaking I don’t think they are writing about the good pain paradox, which is defined by simultaneous pleasure and pain. They are writing about pleasure following pain (relief from pain). This is more comfortable scientific ground: it’s pretty straightforward that relief from pain might be “associated with positive consequences” or lead to “activation of the brain’s reward circuitry,” for instance. Lance a boil, then feel better, right? Of course. But that’s definitely not what we mean by “good pain” in massage. BACK TO TEXT
There are many different styles of massage. The most common is the Swedish massage, which is a whole-body therapeutic massage designed to relax the muscles and joints. Other popular types include deep tissue, shiatsu, hot stone, reflexology, and Thai massage. You may want to choose a specialty, like sports massage or pregnancy massage, if that suits your needs.
In this particular study, published in Studies in Health Technology and Informatics, therapeutic massage included techniques of tapping and friction, while DTM used “oblique pressure and a combination of lengthening and cross-fiber strokes.” All sessions were 30 minutes long and preformed daily, and all participants did not receive any other treatments during the course of the study. After 10 days, participants treated with DTM reported significant improvements in pain (lower back pain in this case) compared to those treated with therapeutic massage, based on scores using the Modified Oswestry Low Back Pain Disability Index, Quebec Back Pain Disability Scale and Visual Analog Scales. The first thing to bear in mind is that deep tissue massage is used for therapy and rehabilitation, not relaxation. So you need to be prepared to feel some discomfort during a deep tissue massage and even after. Always be sure to check with your insurance provider before starting treatment to verify that the massage therapist you want to work with is recognized by your company. You will also want your massage therapist to provide any paperwork you need to submit to your insurance provider.
In 2015 the Australian Government's Department of Health published the results of a review of alternative therapies that sought to determine if any were suitable for being covered by health insurance; reflexology was one of 17 therapies evaluated for which no clear evidence of effectiveness was found.[5] Accordingly In 2017 the Australian government named reflexology as a practice that would not qualify for insurance subsidy, saying this step would "ensure taxpayer funds are expended appropriately and not directed to therapies lacking evidence".[6]
