There’s just no reason to push a client to that “cringe point.” It’s ham-handed, tends to indicate a simplistic “more is better” approach to the work, and simply isn’t needed — that’s not what defines “intensity” in a good massage. Very strong and sastisfying pressure can always be achieved without that edgy, nervous-system-almost-rebelling feeling.
Not everyone should receive a deep tissue massage. Some people simply enjoy the sensation of deeper pressure to their muscles and others prefer a more gentle touch. Someone who has never experienced a massage before may not want to request a deep tissue massage. It is the responsibility of the massage therapist to determine if a deep tissue massage is necessary by way of a thorough health history and evaluation. Couples massage takes place in a room that is, at the very least, large enough for two massage tables, which is the typical set-up in a day spa. On the other hand, resort and hotel spas often create elaborate environments where couples can not only have side-by-side spa treatments, but also take a bath together, get pedicures, spend time in a steam shower, or lounge by a fire after the treatment. 

There’s rarely any justification for extremely painful massage, unless it clearly produces a better result than gentler treatment — which is rarely clear!1 It is possible that a few “brutal” deep tissue massages could do the trick where gentler treatment would fail — but there is no way to know this in advance, and massage is expensive stuff. If you’re going to gamble on a treatment, gamble on cheaper and less painful ones.
Before a session begins, the client will typically be led to the treatment room and asked to remove their clothes and lie down under a sheet or blanket on the massage table. In most cases, and based on comfort level, it is up to the client to determine whether to leave undergarments on. Clients will generally be covered with a sheet for the majority of the session. Those who have privacy concerns may wish to discuss these with the massage therapist before the session begins.
Deep tissue massage uses many of the same movements and techniques as a more classic superficial massage. The difference is that the pressure will generally be more intense, and concentrated on areas of tension and pain. The strokes are the same, but the movement is slower and deeper. At certain points during the massage, most people find there is usually some discomfort and pain. It is important to tell the therapist when they hit a tender spot and if any discomfort you experience is outside your comfort range. Due to the focused nature of the session, the treatment can become intense at times.

The procedure is performed either with the bare hands or through some mechanical means, such as a vibrator. The most common sites for massage are the back, knees, elbows, and heels. Care is taken not to massage inflamed areas, particularly of the extremities, because of the danger of loosening blood clots. Open wounds and areas of rash, tumor, or excessive sensitivity are avoided. Even if the extremities (legs) are not inflamed, they should not be massaged if the client has been immobilized for an extended period of time. In massage therapy, so much can be achieved while inflicting only good pain on patients that bad pain must be justified by vivid, quick, and somewhat lasting benefits — which is a high bar to clear. All health care practices must be justified by benefits. As risk and pain and expense increase, the benefits must also. There is simply no point in tolerating — and paying for — painful treatment without an obvious return on the investment.

Many people are living with chronic pain somewhere in their body every day. Regular and continuous massage therapy can reduce and relieve pain from many or specific areas the body is ailing. Some doctors recommend these kinds of massages as a follow up to a medical treatment, or to be used in conjunction with a larger treatment plan to remedy an injury.
